I Confirm I have completed the age of 18 years prior to my application to MITRA LIFE CARE SERVICES PVT. LTD. (here in after MITRA LIFE CARE SERVICES PVT LTD) and thereby submit my application to the company for consideration.

I shall not introduce or sell any products or service not approved by the Company through (THE COMPANY'S NETWORK) of co-ordinatorship.

My co-ordinatorship business cannot be sold, assigned or transferred to any person without the prior written consent of (THE COMPANY'S REFERRAL CO-ORDINATORSHIP) of co-ordinators.

If I desire to discontinue my co-ordinatorship, I may submit my resignation at any time and for any reason upon giving 45 days written notice to THE COMPANY MITRA LIFE CARE SERVICES PVT. LTD., and I may keep the sale kit but, I Shall not be entitled to any refund what so ever.

In order to maintain viable marketing system and to company with the changes in applicable law. The Company shall be entitled to alter, modify, amend or change the Business Programme from time to time and I agree to be bound by all the terms and conditions, rules and regulations embodied there in for the time in force.

This agreement shall be governed by the laws of the country and all claims and disputes and other matters between parties of this agreement shall be brought to the appropriate court of Law, Bangalore.

The company shall deduct Tax at source from the incentives as per rates prescribed under the income Tax Act 1961. Any co-ordinator found guilty of spreading any sort of misleading information, can be terminated at the sole discretion of the management.

MITRA LIFE CARE SERVICES PVT. LTD., reserves its right to amend rules and regulations & shall make no refund of any kind.

Upon acceptance of my application as a co-ordinator, I shall be know as an authorized co-ordinator and shall be bound by all the terms, conditions, rules and regulations embodied in the Business Programme of which a copy has been shown and read to me by my sponsor prior to my application.

I am responsible to pay all applicable taxes, THE COMPANY provides co-ordinators with monthly statement and annual income statement required by law.

In the event of any dispute arising between the co-ordinator and the company relating to any matter concerned with this appointment or the subject thereof or in relation to and whether as to the construction or otherwise, the me shall be referred for ARBITRATION, as per the Arbitration Act 1940 one arbitrator shall be appointed by each party and two appointed arbitrators jointly shall appoint the third arbitrator shall be final and binding on both the parties. The venue of arbitration shall be subject to Bangalore Jurisdiction.

Any co-ordinator has done any contra activities against the aims, rules and regulations of the company, he would be removed without any notice to him and appropriated action would be taken against him as per criminal & civil laws.

All payments should be accepted in the form of DD only and payable at Bangalore.

MITRA LIFE CARE SERVICES PVT. LTD., shall not be liable for any incidental or consequential damages caused by breach, cancellation or suspension of this agreement, whether or not the possible for such damages is known to MITRA LIFE CARE SERVICES PVT. LTD.,.

For all purpose only the English version will be taken as the official copy. No printed material by club co-ordinators is allowed to circulate without the written approval of MITRA LIFE CARE SERVICES PVT. LTD. otherwise club co-ordinator could be terminated.

This agreement constitutes the entire agreement between the co-ordinator and MITRA LIFE CARE SERVICES PVT. LTD. and no other promises, representation, guarantees or agreements of any kind shall be valid unless given in writing.

The re-admission of resigned/terminated co-ordinator lies solely with the discretion of the management.

I shall act only as an authorized co-ordinator and shall not act as an authorized representative agent or employee of "The Company" in any transaction, nor have any express or implied or extensible authority to bind or make any express or implied representation or warranty on behalf of THE COMPANY.

MITRA LIFE CARE SERVICES PVT. LTD., will operate its own business and will abide all rules, regulations, policies and procedures as amended by MITRA LIFE CARE SERVICES PVT. LTD., from time to at its sole discretion.

Company shall not be responsible for any cash or DD payments made at any place or payments made to any person except for the DD deposited at head office. And also should have a Company receipt.
